Welcome to the website of the Ontario Heart Health Network (OHHN).
The Ontario Heart Health Network is the provincial network that supports the OHHP-Taking Action for Healthy Living initiative.
The OHHP-Taking Action for Healthy Living works across Ontario to promote and support community based heart health and chronic disease prevention programs. 37 Community Partnerships are working towards healthy public policy in schools, worksites and communities. Funding is provided by the Ontario Ministry of Health Promotion and matched in kind by community partners and the hosting agency.

The Heart Health Resource Centre (HHRC) supports the Ontario Heart Health Network through secretariat support, annual funding, and coordinating logistics for Provincial Network Meetings.
