To advocate for the continued funding and support of the OHHP - Taking Action for Healthy Living and to provide opportunities for the planning and delivery of community-based heart health/chronic disease prevention programs.

Environmental Scan
Communications and Key Messages
A communications plan is in place and OHHP Key Messages have been written to educate different audiences on the value of the OHHP-Taking Action for Healthy Living. Evidence is based on evaluation of Phase 1 and feedback from local Community Partnerships.
Community Partnerships are encouraged to talk with their MPPs, Medical Officers of Health and other stakeholders about the importance of the OHHP-Taking Action for Healthy Living to chronic disease prevention. Partnerships are encouraged to use the following tools, and to check out our success stories:

Partnership and Collaboration
The Ontario Heart Health Network (OHHN) is working with other provincial organizations to develop policies and support mechanisms that support community initiatives for chronic disease prevention. The OHHN is represented on a variety of committees e.g. the Ontario Chronic Disease Prevention Alliance.
A representative from the OHHN is available to speak at conferences and events on community-driven health promotion strategies, e.g. “Building Healthy Policy across Ontario Through Community Partnerships”. To book a speaker, contact the chair of the OHHN Executive.
The “Environmental Scan of Provincial & National Chronic Disease Prevention Programs and Strategies” was published in May 2007. This document will be used to identify opportunities for future collaboration, increased partnerships and potential action for the Ontario Heart Health Network.

Membership
The CWG Phase III Working Group was made up of representatives from the Ontario Heart Health Network Executive, the Ontario Heart Health Network, and Community Partners - including the Ontario Chronic Disease Prevention Alliance, the Heart and Stroke Foundation of Ontario, and the Regional Stroke Centres.
The Ministry of Health Promotion and the Heart Health Resource Centre attended in an ex-officio capacity.
